Springfield Public Schools and the Township of Springfield Partner with Rite Aid to Offer COVID-19 Vaccine at Jonathan Dayton High School
May 13, 2021, Springfield, NJ—Springfield Public Schools and the Township of Springfield, in partnership with Rite Aid, are excited to announce a free COVID-19 vaccination clinic for residents of Springfield, as well as employees of Springfield Public Schools and the Township of Springfield. This clinic will offer the Pfizer vaccine to residents and employees ages 12 and older on-site at Jonathan Dayton High School.
Springfield Mayor Chris Weber noted, “This is another important step in restoring the health of our township and our country and I am so thankful for everyone involved in making this happen. Providing this vaccine is a great example of the collaboration between our town and schools.”
Springfield Board of Education President Laura Gamarekian added, “During this challenging year, we have continued to work to support the health and safety of our students, staff, and community at large. We are grateful for the partnership with Rite Aid and the Township as we work together to provide vaccine access to our staff and students.”
The Jonathan Dayton High School clinics will be staffed by volunteers from the district and township, with Rite Aid administering the vaccines. Information about making appointments can be found below and at https://www.springfieldschools.com/Content2/-content-922. The partnership was spearheaded by Apu Mullick, Deputy Emergency Management Coordinator for the Township of Springfield, and Dana Kelly, Director of Human Resources for Springfield Public Schools.
The first dose of the Pfizer vaccine will be offered on Friday, May 21, 2021, and the second dose will be offered on Friday, June 11, 2021. Registrants must be able to attend on both dates, as the vaccine requires two shots 21 days apart. The vaccine will be administered at the Main Gymnasium in Jonathan Dayton High School between 3:00 p.m. and 7:00 p.m.
To register, eligible residents and employees must go to https://tinyurl.com/uakprmjs to make an appointment for May 21, 2021. After registering for the first appointment, individuals will automatically be registered for the second appointment, which will take place at the same time and location on June 11, 2021. Appointments will be available on a first-come, first-served basis.
After registering for the vaccine, a confirmation email will be sent. Individuals registered to receive the vaccine must complete and print the Rite Aid COVID Vaccine Consent form found here https://tinyurl.com/ndhs8btc ahead of the vaccination appointment. Individuals will also be required to provide the following at the appointment:
- Photocopy of state driver's license or state ID card,
- Photocopy of medical insurance card,
- Photocopy of prescription insurance card, and
- If applicable, photocopy of Medicare card.
The photocopies will be collected by Rite Aid. Individuals without health insurance may access the vaccine by notifying the staff at the time of the appointment. There is no cost to receive the vaccine, regardless of insurance coverage.
Only individuals and their identified parent/guardian will be allowed in the building for the identified appointment time. All individuals will be required to follow health and safety guidelines:
- Face masks are required.
- Social distancing rules will be enforced at all times.
- Individuals are required to complete a pre-screening questionnaire by going to https://tinyurl.com/yj6c7wte the morning of your May 21st and June 11th appointments before arriving at Jonathan Dayton High School. You will also have your temperature taken as part of the screening when you arrive at Jonathan Dayton High School.
- If you are in isolation, quarantine, or are experiencing any signs and symptoms consistent with COVID-19, please cancel your appointment so that is available for other eligible community members.
